Accudemia: How do I pull a non-anonymous survey report?

As you may have realized by now, in Accudemia the Survey Reports are anonymous, but we do have a way of figuring out this information by pulling a specific report.  And here are the steps on how to pull that information:
  1. Login to your Accudemia account as an Admin.
  2. Click on the Reports option in the top navigation bar.
  3. From there search for the report called  "Session Log with Surveys Results Incl. Student Data Configuration"  in the left-side search option.

    NOTE:   You can type just a few characters of the name of the report above to find it.
  4. Now set the Report filters for the specific time (1) Period and the specific (2) Survey on the left side of the screen and choose the (3)  Output format on the right side of the screen.
  5. Once done click on the  View Report button to download the results on the next screen.
  6. You should have information like this in the report results (with each question as a new column on the end of the report - but mine only has the one comments field):
    Student Name
    Student ID
    Date
    In Time
    Out Time
    Period
    Tutor/Advisor
    Instructor
    Subject Area
    Services
    Survey Result
